Now Hiring - Clinical Supervisor - Metro, North and South Region


Hours: 8:00am - 5:00pm


The Clinical Supervisor will be responsible for, but not limited to:


  1. Directly supervises all clinical field staff assigned to their team, to include: documentation of competencies and performance evaluations, identification of performance issues, and counseling of employees related to job performance.
  2. Participates and provides leadership for the team for regular IDT meetings.
  3. Monitors documentation of all team members and counsels IDT members when documentation does not meet company standards
  4. Promptly responds to all care and service concerns through direct contact with complainant and provides written follow up to the Clinical Director.
  5. Ensures collaboration with agencies and vendors for effective management of services and ensures services provided by other agencies are authorized by Hospice.
  6. Conducts case conference meetings with hospice personnel to facilitate coordination of care, improve documentation, and ensure that all patient/family needs are met
  7. Conducts supervisory visits to allow for observation for performance of all team members.
  8. Directs staff in performance of their duties, including admission, discharge, transfer revocation and provision of service to patients.
  9. Facilitates problem solving sessions and delivers team support.
  10. Recognizes and performs other duties which need to be performed and as assigned by management



Bachelor’s Degree in Nursing required, Master’s Degree Preferred


Professional Experience:

  • Must have a State Certified RN license
  • Certified Hospice & Palliative Care Nurse (CHPN) certification, a plus
  • Experience in hospice or home health, preferred
  • Minimum of 5 years nursing experience, with 2 years in a supervisory role
  • Demonstrated skills in management performance, including expertise in written/oral communication, supervision of staff, documentation requirements, service coordination and case management.

Must be bilingual, fluent in English and Spanish written and oral communication.
