Now Hiring - Clinical Quality Improvement Coordinator - Grace Health Puerto Rico

 

Hours: 8am - 5pm 

 

The Clinical Quality Improvement Coordinator, will be responsible for but not limited to:

  1. Collaborates with Executive Directors, Clinical Directors, clinical teams, and organizational leadership to implement effective internal auditing plans
  2. Performs audits that focus on compliance and quality issues
  3. Evaluates and compiles data collected during the auditing process
  4. With Clinical Quality Educator, plans, develops, executes and evaluates clinical education programs based on audit findings, survey results, and identified agency needs
  5. Researches and monitors regulatory changes and implements as part of the auditing plan as needed
  6. Ensure adherence to established policies and procedures
  7. Responds promptly to detected concerns, developing corrective action, and reporting findings
  8. Plans, develops, implements, and evaluates Mock Survey process for all agencies
  9. Works one-on-one with employees who have special training needs
  10. Collaborates with Billing and Compliance departments
  11. Assists as needed with any external audits
  12. Assists in identifying areas of weakness for targeted Performance Improvement.
  13. Provides written reports to the Vice President of Clinical Services with the summary details of audit findings to be shared during Governing Body Meetings
  14. Recognizes and performs other related duties which need to be performed, although not directly assigned
  15. Develops plan for all Grace Hospice agencies to have a mock survey yearly
  16. Executes mock survey and develops statement of deficiency based on survey results
  17. Works with each agency, Clinical Quality Educator, and VP of Clinical Services to develop and execute a plan of correction based on the statement of deficiency.
  18. Collaborates with other departments, including HR, Billing, and Compliance, to communicate any findings from the mock survey
  19. Provides written reports to the Vice President of Clinical Services with the summary details of survey findings to be shared during Governing Body Meetings
  20. Supports individual agencies with surveys from external agencies.

Qualifications

Education:

Master's Degree in Nursing

 

Professional Experience:

  • 2-4 years of relevant experience
  • Registered Nurse (RN) license within the state of operations
  • CHPN Certification, preferred
  • 2+ years of hospice experience
  • Expertise in hospice regulations, eligibility, and clinical operations.
  • Understanding of complementary/alternative therapies and holistic nursing principles
  • Bilingual a plus
