Now Hiring - Clinical Services Manager (RN) - North Wales, PA

 

Hours: Monday - Friday 

 

The Clinical Services Manager is responsible for, but not limited to:

  1. Participates and provides leadership for the team for regular IDT meetings
  2. Reviews and revises the patient/family care plan
  3. Completes Performance Improvement projects, as directed by the Clinical Director
  4. Serves as a member of the clinical management team in order to address problems associated with the delivery of clinical services; identified staff development needs; and puts forth suggestions for appropriate changes in policies and procedures
  5. Supervises the maintenance of medical records assuring accuracy, completeness and compliance with regulations
  6. Monitors IDT in completion of care plans
  7. Submits Plans of Correction to Administrator and Clinical Director when required by audit results and/or patient family satisfaction surveys
  8. Monitors documentation of all team members, and counsels IDT when documents do not meet company standards
  9. Promptly responds to all care and service concerns through direct contact with complainant and provides written follow up to the Clinical Director
  10. Conduct supervisory visits to allow for observation for performance of all team members
  11. Communicates all pertinent clinical and administrative information to the Clinical Director and all team members
  12. Arranges team coverage for on-call and staff time off
  13. Assigns patient/family to appropriate staff members
  14. Reviews assignments and revises as needed, in collaboration with the clinical Director to maintain appropriate caseloads
  15. Facilitates problem solving sessions and delivers team support
  16. Identifies needs for staff education and assists in the development of in-service programs for the team
  17. Seeks input and encourages staff to participate in performance improvement activities
  18. Ensures medications and supplies are ordered and delivered in a timely manner
  19. Ensures all staff are properly trained on EMR and associated with all medical programs (i.e. EDRS, pharmaceutical companies)
  20. Ensures proper documentation is completed promptly
  21. Able to cover case management absentees
  22. Prepare End of the Day reports for On Call
  23. Rotation of Administrator On Call
  24. Occasional field visits to assess patient needs

Qualifications

Education:

Bachelor's Degree in Nursing preferred

 

Professional Experience:

 

Registered Nurse

  • 3 years nursing experience,
  • 1 year in a supervisory capacity
  • Prior hospice nursing experience
  • Strong written and verbal communication
