Now Hiring - Nurse Navigator - Bergen, Hudson, Passaic, Essex & Morris Counties, NJ


We are seeking a Nurse Navigator to directly work with a multidisciplinary team to assist patients navigating throughout a Hospice care program.  As a key member of the Hospice Team, the nurse navigator will assess patients for eligibility for hospice services & provide education.  The nurse navigator will collaborate with the multidisciplinary team, including physicians, NP’s, clinical team, and other healthcare providers for care coordination and assessments upon admission into the hospice program.

The Nurse Navigator must have excellent communication skills, utilize clinical knowledge, and critical thinking skills.


Nurse Navigator Essential Duties and Responsibilities:

  • Performs nursing assessments for eligibility for hospice services & works to onboard and consent patients
  • Provides education to patients, families, and caregivers about end of life care
  • Effectively communicates patient updates with members of multidisciplinary clinical team inclusive of physician, NP, intake department and clinical team
  • Performs miscellaneous job-related duties as assigned




Registered Nurse with a minimum of Diploma from an accredited nursing institution.

Preference for a BSN.


Required Skills

Strong communication skills-able to communicate well with multidisciplinary team.

Excellent clinical skills

Strong technological skills

Knowledge of LTC practices, procedures, laws, regulations and guidelines.

Multi-facility management experience is desirable.
