Now Hiring - Palliative Care Coordinator - Marlborough, MA

 

Hours: Flexible part-time hours (5-10 hours per week)

 

The Palliative Care Coordinator will be responsible for, but are not limited to:

  1. Receives NJ Palliative Care referrals via phone, fax, and email
  2. Answers questions about Palliative care from referral sources, patients and families, and the community
  3. Enters Palliative Care referrals into Suncoast
  4. Maintains schedule for NJ Palliative Care clinicians
  5. Maintains highest level of communication with NJ Palliative Care team, Billing team, and HR team
  6. Reviews coding for palliative care visits
  7. Runs weekly timesheet productivity report to ensure that documentation is associated with previous week’s visits
  8. Orders labs, x-rays, and other diagnostic tests for Palliative Care patients
  9. Gathers timesheets for clinicians and implements necessary spreadsheets for payroll approval by Director of Palliative Care
  10. Sends clinical information to referring physician
  11. Discharges patients from Suncoast as directed by Palliative clinical team
  12. Scans and uploads paper documentation into Suncoast Document Library
  13. Assists National Director of Palliative Care with HR functions

Qualifications

Education:

High School Diploma or equivalent, Associate's Degree preferred

 

Professional Experience:

  • Must have a valid Driver's License and a reliable source of transportation
  • Knowledge of Suncoast EMR
  • Must have 2-4 years experience working within related field
  • Have strong communication skills both written and verbal
  • Strong Work ethic and highly detail oriented
  • High degree of computer literacy
