Now Hiring - Clinical Community Education Liaison - North Jersey


Hours: 8:30 am - 5:00 pm


The Community Education Liaison is responsible for, but not limited to:

  1. Through the continuous improvement of standards of practice and by providing a safe and secure environment the customer will be assured a sense of safety and well-being, free from risk and harm.
  2. Establish and maintain professional relationships with all referral sources, including physicians, nursing homes, assisted living facilities and hospital personnel, including but not limited to case managers, discharge planners and other appropriate referral sources.
  3. Transitions patients from hospital or rehabilitation facility to home care services by obtaining patient documentation from referral source. Establishing contact with patient or designated representative when appropriate and providing them with information on Ascend Home Health.
  4. Plans and implements educational in-services and presentations for referral sources.
  5. Makes direct calls on health professionals, including physicians, office nurses, staff nurses, discharge planners and social workers to educate them about services available to their patients.
  6. Continues to maximize opportunities in territory.
  7. Participates in regular marketing meetings. Provides weekly reports to the Executive Director.
  8. Utilizes market analysis data in individual territory identifying new business and potential opportunities.
  9. Participates in marketing events, health fairs, and community events as needed.
  10. Prioritize activities to meet monthly and quarterly growth projections.
  11. Meets or exceeds all growth and development targets and actively establish and maintain market awareness.
  12. Puts customer service first and upholds the Ascend Home Health Code of Conduct.
  13. Ensures that prospective clients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
  14. Has the understanding of Medicare regulations and private duty guidelines.
  15. Continues to drive new business opportunities.



Bachelor's Degree is Marketing, Business Communications or a related field


Professional Experience:

  • 3-5 years experience in Sales/Marketing or a related healthcare field
  • Experience working with professional and non-professional personnel and company representatives
  • Registered Nurse preferred 
  • Self Starter
  • Goal Driven
